SInce 2012
PAHM History
In December 2012, PA Health Management (PAHM) was born. We created PAHM because we saw a need to create positive health outcomes for individuals in our community. In the beginning, we focused on providing Supports Coordination for individuals by being a Managed Care Organizations (MCOs) chosen by the Pennsylvania Department of Human Services to deliver Managed Long-Term Services and Supports (MLTSS). By spending time with each person one-on-one, building supportive teams, and working to understand the details of each unique case, we quickly began to grow. Through innovative ideas, we promoted a higher quality of life for over 5,000 individuals with physical & developmental disabilities, addiction, and the aging population.
