Long-Term Services and Supports
PAHM provides Service Coordination through the Managed Care Organizations (MCOs) chosen by the Department of Human Services to deliver Managed Long-Term Services and Supports (MLTSS). Supports available to individuals are specialised for a wide array of health and social needs, including and not limited to physical disabilities, mental disabilities, brain injuries, cognitive impairments, mental concerns, individuals who are aging, and more.
Managed Care
Community HealthChoices (CHC) Waiver
If you’re 21 or older and have both Medicare and Medicaid, or receive long-term supports through Medicaid because you need help with everyday personal tasks, you will be covered by Community HealthChoices (CHC). CHC will coordinate your health care coverage to improve the quality of your healthcare experience — serving more people in communities rather than in facilities, giving them the opportunity to work, spend more time with their families, and experience an overall better quality of life.

OBRA Waiver
OBRA Waiver, part of the Home and Community-Based Services (HCBS) program, provides support and services to individuals with severe developmental physical disabilities, helping them live independently within their communities. The waiver offers a range of services including personal assistance, home modifications, and specialized therapies, aiming to enhance the quality of life and promote community integration for eligible participants.

Act 150 Program
The Pennsylvania Act 150 Program offers personal assistance services to adults with physical disabilities, enabling them to live independently and participate fully in their communities. This state-funded program provides support such as help with daily living activities, household tasks, and transportation. Designed for individuals who are not eligible for Medicaid, the Act 150 Program aims to enhance the quality of life and promote greater autonomy for its participants.

At PAHM, our focus is on increasing positive health outcomes and the quality of life of individuals in our community. We achieve this by spending time with each person one-on-one, building a supportive team, and working to understand the details of each unique case. We help individuals reach medical and social goals and prevent issues and risks through identifying and addressing their root causes. PAHM is qualified through the Office of Long-Term Living (OLTL), Department of Human Services (DHS), to provide Service and Supports Coordination for eligible individuals throughout the entire state of Pennsylvania.
